| AARP | We are not contracted with the AARP plans. |
| Aetna | We are contracted with all Aetna plans including Medicare HMO, PPO, PFFS. |
| Anthem | We are contracted with all regular Anthem BCBS plans. We DO NOT ACCEPT the Medicare PPO and HMO plans. We DO ACCEPT the Medicare PFFS plans. |
| Cigna | We are contracted with all regular Cigna plans. We DO NOT ACCEPT the Medicare plans. |
| Frontpath | We are contracted with all Frontpath plans. |
| Humana | We ARE NOT contracted with any of the Humana plans. We will accept assignment of benefits for the Medicare PFFS plans. |
| Medicaid | We DO NOT ACCEPT any Medicaid plans. |
| Medical Mutual | We are contracted with all MMOH plans. |
| Medicare | We are contracted with all Medicare plans. We are not contracted with any other commercial Medicare plan except for Aetna. We will accept assignment of benefits for the PFFS plans. |
| Paramount | We are contracted with all Paramount plans. We DO NOT ACCEPT Paramount Advantage. |
| Tricare | We are not contracted with Tricare plans. |
| United Healthcare | We are not contracted with United Healthcare plans. |
All other insurances are considered OUT OF NETWORK. We will collect the payment in full at the time of service. One claim will be submitted to the insurance company. If payment is made on the claim by the insurance company, a reimbursement will be issued to the patient.
