Financial policies
- A current insurance card and photo I.D. are required at the time of service.
- Payment of copay, co-insurance or deductible is due at the time of service.
- We accept MasterCard, Visa, American Express and Discover cards as well as Debit cards and Care Credit.
- We accept checks with proper ID. A $30.00 service charge will be assessed for any returned check. Payment of that service fee and amount of the check must then be made with cash or credit card.
- We will file supplemental insurances when appropriate.
- Balance bill payment is due within 30 days of final payment by the insurance company. Any balance not paid within 30 days will be assessed a 1.5% service charge and will be subject to collections.
- If you do not have insurance, payment in full is due at the time of service.
