Which insurances do you take?
We are contracted with Aetna, Anthem (except Medicare PPO/HMO), Cigna (except Medicare), Frontpath, Medical Mutual, Medicare and Paramount (except Advantage).
What if I have two insurances and you are only contracted with my primary?
We will file a claim with your insurance company. You are responsible for your copay and any deductible at the time of service. As a courtesy, we will file one claim to your secondary, but we will not accept the secondary assignment of benefits, even if we participate with that insurance. You will be responsible for the patient’s portion that is stated on the primary explanation of benefits.
What if I have two insurances and you are only contracted with my secondary?
If you have a primary insurance that we are not contracted with, the total cost of the visit is due at the time of service. We will submit one claim to your secondary. If payment is made by either insurance company, you will get the reimbursement from our office. We do not accept secondary assignment of benefits.
What if I have three insurances?
We do not accept or bill tertiary policies.
Can I be billed for my copay?
All copays, coinsurances, and deductibles must be paid at the time of service. If you do not have your payment available, your appointment will be rescheduled. Any unanticipated coinsurances or deductibles must be paid upon receipt of the first statement. Failure to do so will trigger a 1.5% service fee for the additional statement sent.
What is a cosmetic procedure?
Cosmetic procedures are any procedures that are not covered by your insurance company because they are considered not medically necessary. Examples of such procedures are Botox, chemical peels, microdermabrasions, Restylane, skin tag removals, Sculptra, and removal of benign moles.
What forms of payment do you accept?
We accept cash, check, Mastercard, Visa, American Express, Discover, Debit cards and Care Credit.
What is a Physician Extender?
A Physician Extender is a Physician Assistant or Nurse Practitioner who has trained to be an extension of the individual physician who has trained them. Physician Extenders are highly educated and trained to function independently under the supervision of a physician, and can diagnose, treat and prescribe medications for medical conditions.
What is an Aesthetician?
An Aesthetician is a licensed skincare specialist.
